Parathyroid surgery provides definitive surgical management for selected patients with parathyroid disorders, particularly conditions associated with abnormal parathyroid hormone production. Focused parathyroidectomy is performed in selected patients when the abnormal gland can be appropriately identified. More extensive disease may require bilateral four-gland neck exploration, particularly in multiple endocrine neoplasia (MEN) syndromes, familial hyperparathyroidism and cases of non-localised primary hyperparathyroidism.

Subtotal parathyroidectomy offers another surgical approach for selected patients requiring removal of a significant proportion of abnormal parathyroid tissue. Total parathyroidectomy with autotransplantation to the forearm is also performed in selected cases, including patients with MEN 1 syndrome. Parathyroid autotransplantation allows functioning parathyroid tissue to be placed at an alternative location following extensive gland removal.

Minimally invasive parathyroid surgery is available for appropriately selected cases and can provide a targeted surgical approach when the affected gland has been localised.
